Referring Professional Refer a Patient to Shepherd Hill Referrals to Shepherd Hill may be made by prospective patients, physicians, other treatment programs and professionals, professional peer-assistance groups, employers, court systems, clergy, employee assistance programs, family members or co-workers. The referral source is considered an important link to the patient’s recovery and, where appropriate, will be involved in the treatment program.   When you call with a referral, you may make an appointment for an assessment if you wish. Just ask for our Admissions Office. Assessments will be performed by certified RNs in conjunction with physicians trained in addiction medicine. The physician reviews assessment results, and an appropriate level of care is recommended.     If you plan to make an appointment for an assessment, please have the following information ready when you call: Name, address and phone number of the prospective patient Date of birth and social security number Insurance information (insurance carrier, policy number and telephone number to review benefits) Extent and nature of the problem Name, address and phone number of the referral source.
